I believe that's a screencapture from an old video about sex ed for the mentally handicapped. The teacher had just asked the students to shout out other words for "penis."

I've seen that video during the Anime Hell presentation at Anime North. If I remember correctly its a sex educational for "retarded" people from the 70s.
